"The place you recommended looks perfect. This place has a view of the water. They have fountains (small ponds) in the yard which would be very peaceful and a vegetable garden. Orchids are all over the house so mom would love that and her room has a view of the Sound. They document everything. The menu is very healthy and the place is as clean as can be. Creekside Care Adult Family Home is a mother/daughter team that began in 1996. Provider Pat Nyswonger, lives in the home with her husband and is the on-sight caregiver. Together with additional staffing we provide 24-hour care giving. Pat has received education/training as required by state regulations including First Aid and CPR. Additional training has been received for Nurse Delegation, as well as a Specialty course for Dementia, including Alzheimer’s Dementia and a Specialty course for Mental Health. Pat also participates in annual continued education and is a member of the Kitsap Peninsula Adult Family Home Assiciation.

Staff Nurse and co-provider Dahn Boquist, RN has in excess of twelve years nursing experience of which six years have been with the elderly in long term care facilities. Currently, Dahn is employed part-time in the cardiac ICU of a major hospital and divides her time with Creekside Care where she provides residential assessments, monitoring of resident needs, consults with physicians, nursing/medical advice and staff support.

Currently we have two relief caregivers on staff with extensive care giving experience. They have been certified as nurse assistants and each have in excess of ten year’s experience. Additionally, they have been certified for the Nurse Delegation program. All caregivers receive continuing education annually including re-certification of First Aid and CPR courses.

Creekside Care is committed to provide individualized care to our residents. We are able and willing to serve the following care needs: frail elderly, oxygen dependant, wheel chair bound, bed bound, dementia, cardiac related, incontinence, respite care, DSHS, and the terminally ill.

We would not be able to provide service for residents with the following care needs: insulin independent diabetes, developmental disabilities, dementia-related wanderers, combative and/or abusive behavior that adversely affects other residents or staff.
