 DesMoines Creek Adult Family Home is a state licensed facility and has created a warm, family-friendly atmosphere, with plenty of room for our residents and their visitors. There is even an outdoor play area for the youngsters. Family and Friends are welcome to participate in Holiday celebrations and summer time barbecue's.

Our residents feel right at home in the smaller, more personal setting the Adult Family Home provides. DesMoines Creek AFH maintains a maximum of six residents with two staff members on site the majority of a 24-hour period. This enables residents to enjoy greater individualized care and attention with peace of mind for their families.

Our caregivers are delegated nursing tasks by our On-Call Visiting Nurses.
